Changes to Online Injury Reporting

No one wants an employee to get injured on the job, but if it does happen. Montana State Fund is here to help. Filing a required “First Report of Injury” (FROI) form as soon as an accident occurs can get an injured employee the prompt medical care they require.

Montana State Fund has updated our online FROI reporting tool with user-friendly functionality allowing Montana State Fund policyholders to submit pertinent information in a step-by-step process. New features include:

  • Simplified data entry
  • Streamlined interface
  • Validations to ensure correct information
  • Integrated help functionality (bubble help)
  • Improved navigation capabilities
  • Progress timeline to advise where you are in the process

Logged in users will still have the capability to save information and come back to complete the form.

Advantages of having a user name and password:

  • Allows you to save information and come back to complete the FROI
  • Gives you immediate access to your account information and loss run reports
  • Allows you to report payroll online

How to Get a Username and Password

On the top of the page click create an account and follow the instructions for authorization. You don’t need a username and password to file a FROI.

Please Note: The online FROI tool is only for employers. However, if an employer files a FROI with an injured employee, they can download the completed form and have the injured employee sign it and send it to Montana State Fund via fax (406) 495- 5020 or mail it to P.O. Box 4759, Helena, MT 59604-4759.
